In connection with my voluntary involvement in activities undertaken for and with the participation and support of Hands On Northeast Ohio, a non-profit charitable organization, I hereby agree - on behalf of myself, my heirs, assigns, executors, and administrators - to release, discharge and hold harmless Hands On Northeast Ohio and its officers, directors, trustees, successors, assigns, affiliates, partners, licensees, sponsors, donors, employees, agents, guests and volunteers (the "HONEO Parties") from all claims, demands, causes of action, and suits for injuries sustained to my person and/or property as a result of my involvement in such activities, whether or not resulting from negligence of the HONEO Parties.  
I hereby attest that my attendance and involvement in such activities in voluntary, that I am participating at my own risk, that I have read and understand the foregoing terms and conditions of this release, and that I enter into this release voluntarily.  Furthermore, I irrevocably grant permission to Hands On Northeast Ohio to use any photograph, videotape, videodisc, and/or sound recordings taken of me and any quotations taken from me during my volunteer activities to promote Hands On Northeast Ohio and/or volunteerism, without compensation to me.
I hereby confirm, represent, and warrant that I have never been convicted of or charged with a violent crime, child abuse or neglect, child pornography, child abduction, kidnapping, rape or any sexual offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection with the crimes previously listed.
